SENIOR HEALTH SERVICES

The Howard County Health Department provides evaluation and personal care monitoring services to Howard County residents. Programs include:

Personal Care Program Overview

AERS Overview

Adult Evaluation and Review Services serves adults needing assistance and support and all persons being considered for community placements, nursing homes or for medical assistance funded personal care. Licensed Clinical Social Workers and Registered Nurses keep county residents in the safest and least restrictive environment while fostering independence and personal well being. Services are free of charge.

Services:

  • On-site (home, hospital, nursing home, etc.) evaluation of county residents seeking help with long term care planning.
  • Evaluation of any adult county resident at risk of institutionalization.
  • Guidance to families and linkages with on-going community services.

Personal Care Program Overview

The Medical Assistance Personal Care Program (MAPC) provides personal care services to medical assistance recipients that have a chronic illness, medical condition, or disability that prevents or impairs the accomplishment of normal daily living activities and requires ongoing medical supervision for an indefinite period.
